It is one decision by the voivode that authorises both residence and employment in Poland, so you do not need a separate residence permit and work permit.
The single permit, formally the Temporary Residence and Work Permit, is issued by the voivode (provincial governor) and combines the right to live and the right to work in one document. It ties you to the employer and the conditions stated in the decision, so significant changes (such as losing your job) must be reported to the voivodeship office. It is the standard route for employed third-country workers staying in Poland longer than three months.
